Youth Mission Trip Survey

2015 Mission Trip Participants -

Although a little late, could you please fill out this survey on the summer's mission trip, including seniors and adults. The first 8 questions are specific to planning next year's trip. The last 2 questions are about the youth mission trip in general. (Very similar to last year's questions.)
