Mission Trip Youth/Parents Meeting 4/22/2012


The Youth/Parent Mission Trip meeting this evening was to gather medical/release/health insurance information.

1.) The Covenant Form was presented and discussed.
This must be signed and turned in by all participants. If you were not at the meeting, please download the pdf file on the right, read and agree to it, sign it and place it in Norm Tiedemann's mailbox at church (right column, near the middle.)

2.) T-shirt size
We need to put our T-shirt order form in by May 1 in order to get the Next Step Shirts for everyone in the correct sizes. Please select either Child, Adult Small, Adult Medium, Adult Large, X-Large or XX-Large. Please email your choice to: Maggie Passmore's Email as soon as possible.

3.) Macalester-Plymouth United Church Release/Medical information.
Since Next Step requires emergency contact, doctors and insurance information, the MPUC medical form has been greatly shorten to only ask 2 things that are missing from the Next Step form. If you were not at the meeting, please download the pdf file (Youth Mission Release/Medical Form) on the right, fill it out and place it in Norm Tiedemann's mailbox at church. 

4.) Copy of the youth's insurance card (front and back). Please make a copy of the front and back of their health insurance card and place it in Norm Tiedemann's mailbox at church. 

5.) Next Step Ministries Medical Information and Release
Next Step requires that all participants fill out their online form. This includes:
  • basic information
  • emergency contacts
  • doctor's information
  • insurance information
  • A set of Next Step Responsibilities (Covenant with them)
  • Building skills assessment
To Complete the Next Step Ministries Form - 


On the right side of the screen (Participant Form)  - select “Fill Out Form”

Fill out the first questions with the following information:

            Location:    Disaster Response: Joplin, Missouri
            Dates:        June 17 – 23
            Trip Leader User Name:  mpuc

To complete the form, you will need to know your health insurance, doctor’s, and emergency contact information, last tetanus shot, and you will need to fill out a skills assessment questionnaire.  Lastly, if you are under 18, a parent or guardian will also have to “sign” the release part of the form.

Sunday's Parent/Youth Mission Trip Meeting

Remember: there is a mandatory parent youth Mission Trip Meeting this Sunday at 6pm. Youth, come for youth group 5-6 and then have your parents join in at 6pm.

Watch this space in the coming days for updates, but right now, plan on bringing your insurance card. We may add things so check back prior to Sunday.

The coffee fundraiser was a great success, but we have leftover coffee/tea inventory that that we just can't use. Priced to move! (Actually, same price.)

Tools to bring to Joplin

Youth and Parents -

Here is the list of tools that NextStep requests that we bring with us. Since we are driving, it will be possible for us to easily bring these (and return them.)  Please email Norm (normt@computer.org) or reply to this email if you get the blog updates via email and let me know what you would be willing to let a group of 26 church youth borrow for the week.

Coffee Sales Update

Youth -
A quick update for Coffee Sales –
Peace Coffee no longer has Colombian Dark; so cross that off the order forms before selling to family and friends. The good news – there are still other Dark Roast coffees from which to choose.
The form at the right has that crossed off.
 
Please remember to get this form in to Tanya Bell (or any of the Schafer girls) by April 15th. The coffee will be available for distribution on the 22nd.
